RN, Assistant Supervisor, Home Care — Field and office-based position (30 hours per week) responsible for supporting the Clinical Leadership Team in supervision and education of staff RNs and LPNs, with a focus on case management and assuring patient plans of care and services meet or exceed all quality, outcome and utilization standards. Requires BS and 3 years clinical experience, one of which must be in home health or hospice.
RN, Supervisor/Coordinator, Hospice — FT position responsible for coordination and management of hospice and palliative care services. Management and supervision of the day to day activities of the hospice and palliative care program, including coordination of the IDG and facilitating early transition within agency programs to meet patient goals. Provides staff education, mentoring and supervision of assigned professional staff. Functions as acting Hospice Director, when required. Requires current CT RN and meet qualifying criteria for Clinical Supervisor.
RN, Admission Nurse — Responsible for the initial comprehensive assessment visit, initial care plan development, delivery of initial care, clinical team coordination and completion of documentation at the start of care for agency patients and families in all agency programs (home health and hospice). Periodic patient re-visits and shared weekend rotation. Current CT RN license, home health or hospice nursing and broad clinical experience required. OASIS documentation proficiency required with certification preferred.
Home Care/Hospice Case Managers — Full and part-time, with weekend opportunities. Provides direct care and coordinates interdisciplinary care to group of patients and families. Job share and flex hours possible.
RN, Home Care QI/PI — Flexible, part-time opportunity for experienced RN for clinical EMR document review/processing and staff education. High level of proficiency in standards of practice, regulatory and accreditation requirements in home health care required.
RN, Hospice/Palliative Care — Work from home — Evening/weekend on-call openings for experienced RNs. Triage calls from patients and provide occasional home visits to meet patient needs. Highly flexible scheduling and job sharing opportunities, generous on-call and per visit rates. Additional per diem and salaried opportunities available to meet your needs. Requires CT RN and 1 year clinical experience.
RNs, School & Community Health, Per Diem — Flexible opportunities to provide services at agency
community health programs (immunization/blood pressure clinics, health promotion activities) and
substitute school nursing in the town of Wilton. Requires CT RN and one year experience.
Physical and Occupational Therapists — Salaried full-time, part-time and per diem opportunities to join high-performing rehab team, with particular need for Stamford/Greenwich territory. Requires current CT license and three years experience; home care experience preferred.
IT Director — Lead all aspects of agency technology use. High level of staff interaction and support of large field-based workforce. Ability to lead, problem-solve and manage time and projects to completion. High degree of flexibility, a “can-do” attitude and high level of willingness to support agency’s dynamic, multi-disciplinary initiatives required. Position requires ten years of progressive IT leadership/management experience, LAN/WAN network management, vendor oversight and I/P telephony experience.
Home Health Aides / Homemaker / Companions — Hourly, live-in and shift assignments available. Paid mileage and travel time. Home Health Aide positions require six months recent experience working as CNA or HHA or recent graduate of a Connecticut training program with certificate.
