RNs: Full-time, benefited, part-time, on-call, per-visit and weekend positions. Requires current CT RN license, one year clinical experience.

• Primary Care Nurses / Case Managers: Salaried, weekend salaried and per diem positions available. Competitive compensation and benefits. Provide direct care/case management for adult, homebound patients. Requires CT RN and one year acute care experience.
• Hospice Nurses: Join a dynamic interdisciplinary team providing end-of-life care to hospice and pre-hospice patients in skilled and home settings. Long term care or facility experience a plus.
• Clinical Coordinator/Quality Management: Implement established quality management program using performance improvement (PI) tools and techniques, assist with clinical supervision of professional staff.  Knowledge of PI or home care/hospice and BSN required.
• Intake: RN, LPN or individual with healthcare background.  Process referrals, interact with a wide range of constituencies and provide superior customer service.  Computer proficiency required, knowledge of McKesson software preferred.

HOME HEALTH AIDES: Hourly, Live-in and Shift assignments available. Paid travel time and mileage. Own transportation and minimum of six months recent experience working as a HHA or CNA or recent graduate of a Connecticut training program with certificate.

HOMEMAKER - COMPANION: Provide companionship, safe-guarding and essential homemaking activities for homebound patients. Flexible, per diem schedule.

PHYSICAL & OCCUPATIONAL THERAPISTS: Great opportunities for experienced RPTs to join high performing rehab team—salaried & benefitted part-time or full time and/or flexible per diem visits. Requires current Connecticut license and three years experience. Prior home care experience preferred.

BUSINESS DEVELOPMENT SPECIALIST: Part-time position responsible for generating referrals for agency services by building relationships with physicians, long term care, independent & assisted living facilities and other community resources.  Responsibilities include market analysis, sales strategy development, goals and plans, sales calls and evaluating results and effectiveness of sales activities.  This position will support business development activities and establish strong relationships with new and existing referral sources.  Candidates must be highly motivated and results driven.  Must be able to develop and build strong, lasting relationships.  Excellent planning, organization and presentation skills are critical.  Recent healthcare marketing and/or sales experience required.  Ideal candidate will have established healthcare contacts and be able to readily network within the community.  RN preferred.
